Getting ready for adulthood need not be too worrying! From Year 9 onwards each annual review meeting will have a section where everyone involved in your plan and of course, you, will be able to talk about preparing for adulthood and independent living.

Independent Living
Having choice, freedom and control over your life, the support, and accommodation and living arrangements, including supported living.

The Independence Trust are a service who work with adults with a diagnosis of Autism who need support building a social circle, getting work, getting things to do etc.

Being physically and emotionally healthy
Have you had your 14+ Health Check?
A parent/carer should contact your GP if you have a learning disability or autism. You do not need a diagnosis. Each year you will be invited for a check up to make sure that you are getting the right support for your health and that if you need any medical help, there is a record about your learning disability and how best a hospital can support you (this is called making reasonable adjustments).
In your check up, the GP will check that you are staying well and chat with you what helps you to stay well and see if you have any barriers that people would need to know if you had to go to hospital eg, you may have difficulty with swallowing.
Not all GPs offer this service but the NHS website can tell you the nearest GP who does.

It’s really ok if you are not too sure how you feel about these different areas at the moment, but it is important that people talk to you about what skills you have now and what you might need to focus on so you can live a healthy and independent life in the future.
